Alright, first the background.
I've got 27.5 Stan Hugos with WTB Trailblazer tubeless setup on my new bikepacking rig that isn't even 2 weeks old. I've been on a handful of trips, and the most recent one I rode about 12km down heavy gravel road, fully loaded with gear, running about 35psi. I had a blast and everything handled wonderfully.
On my way up to a trail today with a friend, we were riding the road to get there and same deal, 35psi, same amount of gear, but as I approached a crosswalk some jerkface blew the crosswalk so I had a brake a little hard(was only going 10km/h or so) and then POP! Rear tire popped off and now my rim and tire are in need of replacement.
Any thoughts onto what might have caused my rear tire to blow off?

Likely too much pressure. Check the website for that rim and the max pressure. The Stans on my Powderkeg have a max listed on them of 40. Those wider rooms are likely a lower pressure.
I know that I blew a tubeless setup of my Rovals at 40 (another narrow rim) when initially trying to get them to seat.
Edit: looks like 30max on hugos

Yup, looks like Hugo's are a max pressure of 30psi. Can't believe an extra 5psi would do that, and can't believe I'm so ignorant and went off the max tire pressure

It's hard to believe there would be so little tolerance with the pressure. It would be easy for a gauge to be 5 psi out. I do note that Stan does not recommend WTB tyres.

As others have said, that sounds like too much pressure for a plus tire, even on a loaded bike. Overly high pressure, combined with sudden braking pressure, could cause a tire to blow off a rim if the bead isn't set well.
I have heard that WTB TCS tires don't play well with Stan's rims, but that's just hearsay.
On the other hand, I can attest to the fact that WTB tires play very well with WTB TCS rims. So well that it's all I want to use anymore.

True for 29", but I'm 99% sure that 26 and 27.5 are copacetic.
